An important aspect of the program is appropriate Case Tracking of the injured worker. The worker’s case must be handled to ensure that all Worker’s Compensation forms, authorizations, documentation, consults, referrals, and follow-up are completed in a timely fashion.
Communication with the employer is a vital component of case tracking. We offer employers the option to access their employees’ medical reports through a secure portal. Through the portal, the employer will be able to see real-time documentation of each visit. If an employer chooses, they can continue to have written rerports mailed, emailed or faxed.
Login to the employer portal here. Contact Sami Evans to set up your Employer Portal: sevans@sterlingm.net or 1-435-265-6574.
Components of case tracking include the following:
- Overview of Case Management and Post-Injury Response
- Prompt evaluation, treatment and/or referral of work-related injuries
- Timely communication and feedback
The communication between Sterling and the employer starts when an injury is identified by the employer and reported to our team.
The Case Manager will follow the injured employee through all the evaluation processes in the Urgent Care and facilitate any appointments to specialists if needed.
Further, the Case Manager:
- Facilitates the employee’s care through respective treatment.
- Compiles all necessary data for the employer.
- Gives aftercare instructions to the employee with appropriate follow-up when necessary.
- Contacts the employer’s representative with an employee status report and recommended work restrictions.
- Sends a report to the employer that can be used to facilitate filing a claim. Patients can view their health records here.
- Summarizes the individual cases and makes recommendations for case resolution.
- Reports all findings to both the employer and the insurance carrier.
Role Expectations and the Case Manager
Each Case Manager will become acquainted with:
- Contact person
- Type of products
- Worker’s Compensation experience
- Specific risks within the work environment
- Special needs for medical surveillance programs
- Temporary work assignment availability
- Polices regarding work-related injuries
- Company preference regarding feedback from the Sterling Occupational Medicine Program
The Case Manage will follow through until the client refuses therapy/treatment, returns to full duty or has their Worker’s Compensation claim denied.
Documentation of all interactions will be provided through a secure site for employer access to the electronic medical record.
The Case Manager will work with the employer to establish goals for each case. The goals will be reviewed and re-evaluated on a weekly basis by the Case Manager.
The Case Manger will seek Company feedback and worksite visits will be conducted periodically for clients.
